Affiliated Radiology<\/strong> is a new national network formed by two independent US radiology groups \u2014 Colorado Imaging Associates (CIA) of Denver and TRA Medical Imaging of Tacoma \u2014 to share PACS<\/strong>, artificial intelligence infrastructure and read capacity without either practice selling equity to outside investors. Announced on September 9, the venture starts with nearly 200 radiologists and an explicit invitation for other independent groups to join.<\/p>\n

What Affiliated Radiology actually is<\/h2>\n

The entity describes itself as a physician-led radiology services organization. In practice it is a shared layer of technology and operations: a scalable PACS platform, AI-supported workflows and clinical AI diagnostic tools running on common infrastructure. The pitch is to pool technology investment, purchasing power and clinical scale across several groups so that expensive tools cost less per study \u2014 the release specifically names AI-supported pre-drafting<\/strong> of reports \u2014 and to pass those savings straight to the partners.<\/p>\n

The second pillar is clinical. Affiliated groups share read capacity, subspecialty expertise and coverage under coordinated standards while keeping local ownership and physician leadership. On its website, the network lists the subspecialties where it intends to harmonize coverage: neuro, emergency, musculoskeletal, body, breast, pediatric and cardiothoracic imaging. Coverage can be delivered locally, regionally or through secure teleradiology<\/strong>, depending on what each hospital or imaging center needs.<\/p>\n

“This partnership allows us to combine the strengths of multiple radiology practices with the technology and clinical scale of a much larger organization,” said Josh Bryant, M.D., president of Colorado Imaging Associates. “We can share infrastructure, expand access to AI, and support one another clinically without sacrificing local ownership or physician leadership.”<\/p>\n

Who the two founding groups are<\/h2>\n

Colorado Imaging Associates has been operating for more than 50 years, employs about 50 radiologists and seven physician assistants, and reads roughly 600,000 studies a year. It covers every CommonSpirit Health<\/strong> location in Denver and western Kansas, plus other hospitals and imaging sites across the Front Range and Rocky Mountain regions.<\/p>\n

TRA Medical Imaging is older still, with more than a century of practice in Tacoma, Washington. It employs around 135 radiologists, stretches geographically from Seattle to Olympia and handles about 1.7 million studies annually, according to Radiology Business. The group is physician-owned, runs four outpatient imaging centers and serves hospitals of MultiCare and Virginia Mason Franciscan Health \u2014 which has been part of CommonSpirit since the 2021 merger of CHI Franciscan and Virginia Mason.<\/p>\n

That is the practical link behind the alliance. Both groups already serve the same health system in different corners of the country. For CommonSpirit, the network deepens the bench of available radiologists; for the groups, it creates the scale to negotiate and invest together.<\/p>\n

“Technology is fundamental to how radiology is practiced today,” said Douglas Seiler, M.D., physician president of TRA Medical Imaging. “Affiliated Radiology gives groups the scale to access sophisticated PACS, reporting and AI infrastructure while connecting them to a broader network of radiologist expertise and clinical capacity.”<\/p>\n

An alternative to private-equity consolidation<\/h2>\n

The model matters because US radiology has spent a decade consolidating. Radiology Partners<\/strong>, founded in 2012 and now above 4,000 radiologists, has just agreed to buy Everlight Radiology for roughly $715 million<\/a>, extending the platform logic into global teleradiology. RadNet, publicly traded, keeps acquiring imaging centers. In both cases the capital comes from outside the practice, and return targets come with it.<\/p>\n

Affiliated Radiology is trying to capture the gains of scale without the change of control. It is not the first to try. Strategic Radiology, a coalition formed in 2009 that now gathers 50 independent practices and more than 2,500 radiologists, runs on group purchasing and benchmarking \u2014 and in August it opened NewVue’s orchestration platform to its members<\/a>. The design difference is that Affiliated starts with shared operating infrastructure \u2014 one PACS, one AI layer, cross-group reading \u2014 rather than negotiating leverage alone. It is one step further in integration while ownership stays distributed.<\/p>\n

There is a cost to that. Sharing a PACS across groups in different states means standardizing protocols, exam naming, worklists and case-distribution rules \u2014 exactly the kind of problem that cloud teleradiology platforms<\/a> try to solve when reading is distributed. It also demands clear governance over who reads what, how quality is measured and how revenue from cross-read studies is split. The announcement does not address any of those points.<\/p>\n

What it means for independent groups elsewhere<\/h2>\n

Outside the United States, the same pressures are arriving with a lag. Hospital systems in Europe, Latin America and Asia want broad subspecialty coverage, fast turnaround and consistent quality, and single practices often cannot deliver all three at once. The usual answers have been to sell to a larger operator or to outsource nights and subspecialties to a teleradiology vendor. The operating consortium is a third option: groups keep their legal entities, brands and local contracts while sharing PACS, AI licenses and subspecialty read capacity.<\/p>\n

Part of the infrastructure already exists. Teleradiology operations that scale on a single cloud PACS<\/a> have solved the technical side; what is missing is the arrangement between equals, where none of the parties is the acquirer. The regulatory work is heavier than the technical work: cross-border or cross-jurisdiction reading requires licensing in each market, data-sharing agreements between separate data controllers, and per-site access control inside a common archive.<\/p>\n

The economics are the same everywhere. Report-drafting AI, detection tools and modern PACS are priced by volume, and unit prices fall with scale. A group reading 600,000 studies a year pays less per study than one reading 60,000. Ten groups of 60,000, together, negotiate like one of 600,000.<\/p>\n

Outlook and open questions<\/h2>\n

Affiliated Radiology says it will pursue further scale in shared services, infrastructure and administrative functions to cut cost and complexity and keep partners focused on the communities they serve. The real test will come with the third and fourth groups: how many independents will agree to move their PACS onto common infrastructure controlled by peers, and how the network will handle conflicts when two members compete for the same hospital contract.<\/p>\n

On that, and on revenue sharing, the release is silent. But the move lines up with another recent signal: the exodus of radiologists from the Veterans Affairs system<\/a> shows that the US bottleneck today is people, not software. Networks that share radiologists \u2014 not just licenses \u2014 answer that directly.<\/p>\n
